This form must be completed by all organizations or individuals in the US and Canada that want to get access to Military Critical Technical Data. According to the US/Canada Joint Certification Program, only certified contractors can get access to unclassified technical data. The form DD 2345 helps the organization to become a certified contractor.
What is the DD form 2345 for?
This agreement certifies that the contractor agrees to the terms and conditions listed in the form. The Military Critical Technical Data can’t be disseminated to other individuals and the certified contractor must not violate this rule. If the Joint Certification Office approves the agreement form, the contractor can get access to the Military Critical Technical data.
What documents must accompany the form DD 23345?
Together with this form you should send your company’s incorporation certificate, state/provincial business license, sales tax identification form or other documents that certify the legitimacy of the company.
When is the DD2345 form due?
The approved agreement is valid for 5 years. The estimated time of completing the form is 20 minutes.
What information should be provided in the fillable 2013 DD 2345 form?
The applicant must provide the following data:
- Type of submission (initial submission, revision, 5-year renewal)
- Name of company or individual
- Name of subsidiary/department
- Cage code
- Personal information of the custodian (name, phone number, title, email)
- Detailed description of business activity (the applicant has to show that the access to the military critical technical data is very important for the business activity)
- The applicant must also sign and date the agreement.
- Once the certification is accepted, the contractor receives this form back with a 7-digit certification number. This number identifies the company as a certified contractor.
What do I do with the form after its completion?
The company has to send the completed form to the US/Canada Certification Office.
